Actually, that page is from the NicoNico Douga dictionary, so that may not be really a reliable source of info. Just my two cents.

It appears to be reliable. There's a screen capture of the closed captioning in this blog and she's called "Tachibana" there.

I was skeptical of the various posts I found originally that essentially had fans saying the same thing - asking if the subtitles could confirm it or if the seiyuu knew anything, etc. That's why I went to try and find secondary source and found matching info in both the NND and Pixiv dictionaries. Considering how on-top of things the K-on fans among the users of these sites are and how into GWSNTY they've been, I figured it'd likely be accurate and up-to-date information. I can definitely understand it potentially not being reliable enough, though.

EB beat me to the punch in locating a screenshot by just a few minutes, it would appear, but I now have a different one taken of a 2ch user's screen that shows the same caption. That, coupled with the numerous sources and posts that mention her being referred to as Tachibana in the episode's captions, seem to make it pretty reliable.

And just to be thorough, this is the blog entry in Nakamura Tomoko's blog where she confirms she is the seiyuu for the role, though she refers to her by her GWSNTY alias. Whether Nakamura has stated anything more than that is beyond me, so perhaps the 'confirmation from the seiyuu' in the OP was a bit of a misinterpretation on my part. My apologies.
