O Medal is basically a general description for two types of medals: core and cell.
Cell are a manifestation of a human desire containing power and make up the body of a Greeed and Yummy (Yummies being the result of that desire). While Cells lack the power of a Core, since they have limited uses, with enough cells a crapload of them can make up the equivalent of one core. Though each cell correspond to a different animal or creature, there's no difference between one cell to another. The Kougami Foundation utilizes cells for their technology: the Candroids, Medajaribur, Ride-Vendor, and Kamen Rider Birth. In Birth's case Cells are used to transform and access various weaponry. Unlike OOO Birth doesn't require a specific Cell Medal (though all we see him using is the Sasori cell) but he does need to carry numerous cells (they're like ammo).
Core Medals are a kind of artificial life that makes up a Greeed. Each core correspond to a color that represents a Greeed: Ankh-Red, Uva-Green, Kazari-Yellow, Blue-Mezool, Gaamel-Grey, Oda Nobunaga-Black, Maki and Eiji-Purple, Shocker, Imagin, and Orange (Fourze and OOO's Movie Wars movie has more cores but I haven't watched it yet and DVD's out May 21st). Cores come in three types: head, arms, and body. Cores are manifested desire of a Greeed (except the Dinosaur medals since they're the anti-thesis of desire) and their modus operandi is to regain their 9 cores to feel "whole." Hino Eiji/Kamen Rider OOO utilizes them to transform using various combinations, with the same color set making up a Combo. A Combo holds massive power compared to a random combination
In the show Ankh compares core and cell medals to a popsicle. Cores are basically the ice while cells are the stick.
