If you think of a first-person 3D view which you can use the mouse to look around with, W/S for moving up/down and A/D for moving left and right, then you have the left and right mouse buttons free for forwards/backwards. Perhaps a combination of a 3D view and a top-down 'radar'-style view where you can see it from the more classic 2D view would make it more feasible.
It would take a long time to get used to (and probably to code) and the gameplay would probably be better original 2D-style, but I'd still really love to see shining danmaku fly past me in 3D leaving smokey trails which you could fly through. Could make good use of HDR too...
