It's funny you say that right now, given that Wikipedia article is debating whether it was used as a device torture or simply as a gag (device to restrain speech). But given the previous strips, yeah it's a torture device here.

So it's not the tool used by gynecologist.

I find the notion that people would argue that a metal device rammed into your mouth (if you're lucky) which forces your mouth open (with spikes in some examples) so you can't scream for what is coming next not being torture a somewhat strange one.

It's like saying, "Oh no, no, this isn't torture, we're just pulling the skin off him to help him stay cool during the summer months."

I'm aware for the context here is in fact a torture device, but if you read your own linked article you'll realize the only ones with spikes are the ones shown at museums dedicated to torture without other proof of its use or existence. And that's exactly the situation with the wiki entry, you just don't describe duct tape as a torture device because when put on face it obstructs speech and breath. You don't describe a glass bottle as a masturbation tool because some sick people force it in the anus. In the same way you don't describe a drill as a torture device only because some psychopath drilled holes on other human skin.

Another curious thing is despite all the torture shown here, Aoba is shown barely unharmed. And so far, while still untranslated the comic, it seems all the methods have been used to intimidate rather than torture. Except for the acupuncture thing which was a method Shiranui used to get answers from her, the burnt body looks more like a dramatic shadow to me. Everything so far has been really ambiguous.

Honestly, I'd argue back using the same logic: A baseball bat can be an implement of torture even if it isn't commonly used as one if you use it to smash people's kneecaps and arms. Baseball bats are a device of torture if you use it for that purpose.

The thing is, the pear is used only in the context of torture, while duct tape and baseball pats have more mundane purposes. The table that a surgery takes place upon, even if it never makes an incision itself, is a tool of surgery, as well.

As for the previous image, yes, that was almost certainly a dramatic shadow (like a censoring pixelation, just to avoid showing the actual brain being poked).

Maybe Shiranui has access to a few repair buckets or the dock, and is just prolonging this?
