Duckie works very good. I'm worried that people unfamiliar with Cockney might not get it, but eh, most people should understand that the diminutive can be seen as a put-down insult, so go ahead.
Granted, it doesn't have the gambling connotations of sucker, but well, can't think of anything better here.

I dunno, I've renchan'd plenty of times. Pisses the hell out of the other players, but it's doable. Never going negative is possible in a 4-player game if you can bait the others into going risky.

Generally impossible, as nearly all variants of mahjong (or all of them) are zero-sum games (ignoring rounding errors, which are negligible). Even penalties are paid to other players.
I would be like if all players lose money at poker.
Of course, the sheer amount of misfortune concentrated there may warp reality anyway...
