I remember some freaky manga where a girl was in a circus and the circus owner had a thing for licking people's eyeballs. I've never tried it myself. I'm considering it anti-romantic for the time being.
As for the picture, the artist likely meant Marisa to be licking away Alice's tears, but the placement is off. Admittedly, it'd be extremely difficult to get such placement right.
