It looks more like a diversion maneuver than a trick... And a really well applied one at that.
By exploting Hamahama's self-conscious weak spot he left her wide open to an all out frontal attack (as it has been mentioned before my comment).

Headbutting won't work from her position and groin kick needs her to be on a position to let her be able to do so, Teitoku is on top of her judging from the position of his arms.
