Enh, playing some of my space-sim games like X3, I hold my controller in reverse monkey-grip (controller upside-down, index finger on the left stick, thumb on left shoulder buttons (throttle), ring finger on face buttons (confirm or go back a menu), and pinky on right shoulder buttons (yaw) - I jam it against my leg to keep it steady, and play with one knee up) so I can dodge lasers and adjust throttle with my left hand, while I use the keyboard to send orders to my fleet at the same time.
