I don't see why blacklists should be an argument for or against flags. I'd say the ToS (or in other words: Rules) is extremely clear.
Even guro is a frequent flag subject and this tag is on the blacklist by default. In other words, blacklist are no reason at all since the site supports that on some tags by default and the posts under these must meet a higher quality threshold (at least Guro). And besides, "grotesque" (look up the ToS if you want to see what this term refers to) is no tag and can't be put to the blacklist list.
Unless you want to exclude a larger portion of posts from your search then you can indeed use blacklists.

I was going to put forth the suggestion that the character is well drawn enough to be tasteful and the image has not crossed the borderline to grotesque, since the breasts don't even come close to exceeding 3 head sizes... but I guess it's too late for that input since the atmosphere already dove straight off the cliff at mach 6.

Her body is for one pretty small and overwhelmed by her breasts. One could exaggerate and say that her body is the breasts and that wouldn't be all that wrong.
That turns out that the breasts are still extremely huge even if they are only one tiny bit bigger than the head. That's just abstruse anatomy and not "tasteful" at all. The "grotesque" clause still fits here, even when it's not the head/breasts relation but more the torso/breasts relation.
There is a detailed rejection message that debunks the flag reason as well, but points out other problems. A flag reason doesn't need to be correct. That doesn't mean you can write every bullshit in a flag reason. But if it's incorrect then it gets discussed and maybe another reason pops up.
I think that's what most people think: If a flag reason is insufficient, lazy or wrong that the post should get approved. But that's not correct since not the flag gets judged but the image per se.

On top of that, the ass fangs do look really bad. They start at the middle of her thighs.

Overall, I conclude for myself that this is rather low quality and should probably get deleted.
