Patchouli's asthma is described by ZUN to be self-inflicted, not a genetic weakness. She has her asthma because she spends all her time in a musty, moldy library with poor ventilation in a humid environment (by the lake) that helps foster mold growth. Her general weakness and anemia is caused by malnutrition because Patchouli doesn't bother to interrupt her studies or writing even to eat. None of that is necessarily genetic, but lifestyle-dependant.
It depends more on which parent's personality or lifestyle she more chooses to take after, although I'd expect she'd be more active and adventurous and likely to take basic bodily maintainance measures than Patchouli, at the very least.

I was under the impression that after completing a certain magic which grants them their long life, touhouverse magicians did not *have* to eat?

Patchouli was born a (long-lived) witch species. That spell is for humans who want to become a witch.
Her little bio from re-reading the Touhou Wiki from EoSD says she was actually born asthmatic, so I guess I might be wrong on the "asthma is caused purely by the dust", but I remember reading that elsewhere, as well.
Patchouli is noted as having a vitamin deficiency, and when she fights Marisa the first time, Marisa suggests that she's full of nutrients, so Patchy should try to eat her, and so Patchouli tries to look up "how to get rid of the unneccessary parts of your food".
---- Anyway, thinking about it, Marisa is not described as having natural talent (but not described as not having natural talent, either), so Marisa's powers are all the result of extreme hard work, which she tries to mask behind her cavalier attitude. Patchouli is focused and has a deep love of books and magic for their own sake. Marisa is focused and daring and energetic. Maybe their child will have a Youmu-like personality of extreme dedication (either to learning magic or her family) and seriousness to the point where she doesn't quite think out the things around her?
... I kind of want to make some of these "If they mated" characters, myself, but would be more interested in making their powers and personalities than necessarily drawing them. I'd kind of see a Marisa/Patchouli child as a red-headed mage... As I've been thinking about it, I came up with her being too young to really play with spellbooks, so she's an artist instead, and makes magic scribbles that can have quick effects.
Name's a bit hard, I don't like these names made of just mashing two names together... So I figured I'd go with a flower name, since Patchouli is the mother, and Marisa's name is possibly supposed to be either western or eastern, and took the flower that means "Sincerity" - Anemone.

It's romanized Kirisame Marisa, family name before given name, a clear sign that her name is eastern. It also written in kanji instead of katakana.
For the child, how about "mugwort" in Japanese: "Mogusa"? Mugwort seems like a more magical type plant then the anemone, and "Mogusa" ending in -sa makes a nice connection to Marisa.
Anemone in Japanese: Isoginchaku. What a terrible name.
