This is mostly likely a time paradox. Seriously.

And on some off-shoot chance that did somehow had a child together, the Remillia vampire genes probably don't kick until the little kid is hungry.

I took a look at the blog post that accompanied this image and it confirmed that the child is simply a young Sakuya and unrelated to Remilia. This makes sense within the context of the artist's comics.
