Machine translation of the commentary comes out to "the day Flandre was born," but I'm not sure whether that just means it's her birthday, or should be taken literally. In the latter case, I guess this artist is taking the view that Flan was born human and made into a vampire later.
(... You would think that would've happened at night, though, wouldn't you?)

Are we to assume this image is implying Flan isn't actually Remilia's biological sister but instead became a part of her family after contracting vampirism?
Oh, I see. I didn't notice that the first time. I was struck instead by the fact that her arm is bandaged for some reason. And wondering why Remilia is crying. The details here raise a lot of possibly-unanswerable questions.

Wouldn't that also explain why she has overly top powers? or how she was closed off the outside world in the basement? or how she got her wings?... or how is she a blonde?

Well, we don't know what their parents looked like...
To me this feels like it's implying that flan was a friend that became as close to a sister as she can get that Remilia made that she couldn't live without and gave the "Gift" of vampirism to that was injured or possibly attacked because of said friendship hence the tears because she knows not everyone would like said gift.
