I think he humped himself to death on the pillow.

Note the wide open/broken window, torn curtains, that poster half tacked on, and the fact that the girl is blond, so are the girls on the pillow and the magazine on the floor. Also note the drawing pad.

What I think happened here is that the guy was obsessed with a character, the blond. So when he found out that she was based off of a real girl, we wanted the 2-d girl to come to being... so he killed her 3-d model and stuffed her body in the pillow case of the 2-d girl in hopes of his only 2-d love to come to life.

If I'm recalling the correct mythology, is Sumerian/Meopotamian myth death was supposed to be caused by it comming to you in the form oa woman so beautiful you essenitally orgamsed out of your body. Been a loooonnnnggg time since I read it, so I've probably got something wrong in the details, but maybe that's what's happening here...and the bloodstains are a result of him essentially orgasming his soul out of such a tiny opening.
