Pretty sure it's shopped, looks like it's her pose from episode 12 when she greeted Kyon and his friends during the school festival. She had on a maid outfit then.
I wouldnt call it shopped... looks more like that person redrew the whole scene... its evident from the colouring style of the background and customers ^^
