This is related to two Doujins by Personal Color, "The Young Girl and the Little Girl of Moriya" (about Sanae) and "The Young Girl and the Little Girl of Hakurei" (about Reimu). In a nutshell, Marisa spills a potion that makes any human regress physically into a little kid. The previous two doujins featured Kanako and Suwako taking care of Sanae until the potion wears off, and Yukari and Suika doing the same for Reimu.
If you follow the logic, this means Alice does the same for Marisa.

Another doujin in this storyline has come out, this one having Alice care for Marisa. I'd been thinking the cause of it was one of the three common explanations listed above (possibly in a future setting, since Alice looks like an adult in it), before you reminded me of the others.
