It's called a pear. You insert it into the vagina, and as you twist some screw thing somewhere on it, the top "petals" (don't know what else to call it, but that's what it looks like in this picture) expand inside of you and causes mutilation. :D If you actually die though, it's more likely to be from infection if it wasn't cleaned the last time it was used.
