This has to be deliberate. Even if it used AI, look at the hands and feet and overall style, or look at the brick work: if AI was involved, this image must have been redrawn almost from scratch. The artist is more than competent enough to notice the bad anatomy and reroll. Looking at the artist's other works and the Pixiv comments, I think the commissioner specifically requested this as some fetish.
