I think she's a little to enthusiastic about that "accident" for it to be one. I think it's pretty clear she knows full well she's doing it wrong.

...You DO know who Yuuka is, right? Playing 'innocent sadist' doesn't mean she doesn't know what she's doing. It's just what she does. Hence why they're so afraid of her - she's doing it on purpose, and is enjoying the heck out of her. A treat for her is a trick for everyone else.
