He actually gets it twice.

Once, at the end of game when he looks up at Aeris/Tifa, and the whole ending I'm pretty sure he's got one and it looks pretty fresh. Probably obtained from Sephiroth fight.

The second time he gets in THE SAME FUCKING spot, is from Yazoo during the first encounter between him and the triplets. When Yazoo shoots off his goggles he gets nicked there.
