Madoka only appears for two reasons, and regardless of which one it is the end will always be the same.

Exactly. It's not just 'Almost' when you see a Kyubey Mogu Mogu. Heartwarming to the max. Unless what you meant 'Almost' was 'Almost 100% heartwarming, but 50% heartwarming, 50% non-selfish hilarious'

It is said that the scriptwriter realized that the earthquake would spin the originally-planned ending into something too depressing even by his standard and therefore decided to place emphasis on the "Becoming Hope Itself" theme.

It's certainly open to interpretation one way or the other. If you want to believe Madoka is the harbinger of death for magical girls, that's fine, but if you'd rather believe she's escorting their souls to a place where they can find eternal happiness, then that's fine too.
