"What's the purpose of life if you can't die?" When the subject of immortality comes up, this is a common sentiment. As if people only enjoy anything because they're constantly aware that their time is limited or something.
The hourai elixir affixes the soul to the body and rejects any change made to both. If that's the case, it might be that it's biologically impossible a hourai to develop insanity. Especially because Mokou still retains complete sanity even after nearly a thousand years of nothing but slaughter and boredom and hatred against Kaguya. It's all fan interpretation though.

One could also interpret that Mokou and Kaguya have actually gone through fits of Depression and insanity for the long years they have been alive and eventually got over it. Becoming stronger(mentally) in the process.

She did go insane, though. For 300 years she went and slaughtered everything and everyone in her path, youkai and human alike in her vengeance for her exile. Even that died in time, though, and she grew bored for the next 300 years. Then Kaguya came and everything got exciting again for the next 300. But then even that died after a while, and so now she's simply a forest guide and escort. It's funny just how long forever is. You'd loose your sanity, but it'd be so long that you'd eventually loose your insanity too, so you become sane again. I think this is what happened to Mokou as far as that goes. I don't think the Hourai Elixir has any partakes in biology, though, or anything else besides the actual consumption.
I'm not sure. As far as I remember (correct me if wrong), I don't think insanity was mentioned in those three hundred years. What I remember is that Mokou was so overcome with boredom that she simply couldn't sate it any other way than killing everything she saw, only to lose interest in even that. I think she did say it was 'maddening'. (Cage in Lunatic Runagate. Really got to check for myself, but....)
Oh and regarding her body, this is what ZUN mentioned in a Touhou popularity poll her participated in. He gave a vote to Mokou (yay!) and commented: "What makes one a human is the mindset of being a human. The DNA comes after that." So Mokou's body indeed is no longer biologically human.
