Was there ever really a female Captain America, or is this just a gender_swap?

Not to my knowledge so yes, this would be genderswapped.

There was a female Captain America in the ill-advised Marvel Mangaverse. Then again, there was also a female Punisher who dressed up as a bondage Geisha who spanked and tickled criminals instead of killing them, so... yeah.

There was also a female Captain America in the alternate future timeline "Marvel M2". In this timeline she was trained by the original Captain and took his place; she called herself American Dream.
