Just at how the 4th couch represents the present, I think the 5th represents the uncertainty of the future. How is all this going to affect Luffy's future?

No. The Healing Hormones were the only ones who were stated to have that side-effect, and the only time he was injected was in Impel Down to save him from Magellan's poison.
Healing Hormones =/= Vigour Hormones
The Vigour Hormones are the ones which he recieved twice, and they don't have such side-effect, they just fool the body into ignoring its own damage/pain while givig a boost of energy. The only stated side-effect of this type of hormones, is the fact that once the hormone's effect wears off, he will feel all the damage/pain he accumulated, all at once.
